Continuing Education "By Mail"
Prefer to order your credits by mail? Simply print out the attached "By Mail" order form, select the courses you want to take, then mail, fax, or phone it in to the Optical Training Institute. For each course you order you'll receive a printed and bound workbook, a set of questions and an answer sheet. When you complete your answer sheet mail or fax it to Optical Training Institute. We will grade it within 2-4 working days, and with a passing grade of 80% or higher we will mail you your ABO/NCLE or state board certificate. ORDER FORM
